A familiar moment
The daughter trying to arrange more support at home
You are supporting another person and family when a daughter calls about her parent, whose needs have changed. Ziverly can capture who is calling and their stated relationship, who needs support, broad location, current care, GP, district-nursing, hospice or specialist-palliative involvement mentioned, the kind of practical help the family is exploring, preferred communication, language or accessibility needs and a safe time to respond. It does not request a detailed medical history, diagnose symptoms, assess capacity, consent, prognosis or care needs, advise on medicines, pain, nutrition or equipment, provide clinical triage, confirm eligibility, staff, funding, availability or a care arrangement. Severe pain, breathing difficulty, uncontrolled bleeding, sudden deterioration, immediate safeguarding concern or another urgent clinical situation requires the approved emergency, NHS or existing clinical-team route rather than routine follow-up.
