You are completing a home session when a daughter calls for a parent recently discharged from hospital. Ziverly can capture who is calling and their stated relationship, the broad reason for seeking a home visit in their own words, location and access, communication or mobility context volunteered by the caller, existing GP, hospital, community or rehabilitation involvement, referral or discharge information they already hold, preferred contact and broad timing. It does not assess symptoms, pain, mobility, falls risk, rehabilitation needs, consent, capacity, suitability or urgency, give exercises or clinical advice, interpret a referral, collect unnecessary medical history, quote, book or promise availability. New or severe symptoms, breathing difficulty, suspected stroke, serious injury or another urgent medical concern needs NHS 111, 999 or the caller's approved clinical route rather than routine follow-up.
